Aliya Healthcare is seeking a dedicated Registered Nurse or Licensed Practical Nurse (LPN) to join our skilled nursing facility in Chicago. You will provide total nursing care to residents, supervise team members, and ensure compliance with facility standards.

This role emphasizes compassionate, evidence‑based care, coordinating with the healthcare team, administering medications, and supporting resident well‑being during the 11pm–7am shift.

Qualifications

• Graduation from an accredited nursing education program.
• Active Registered Nurse or Licensed Practical Nurse license with Illinois Department of Public Regulations.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Physically able to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Registered Nurse or Licensed Practical Nurse (LPN).
• Participate in developing and implementing individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
