Job summary

ALIYA Healthcare seeks a compassionate Registered Nurse (RN) to provide total resident care on the assigned unit in our skilled nursing facility. You will collaborate with a multidisciplinary team to develop and implement care plans and administer medications per policy.

You will lead by example as a charge nurse, ensuring safe care delivery, documentation, and adherence to all safety protocols while supporting ongoing staff development.

Qualifications

• Active RN license in the state of Illinois required.
• Graduation from an accredited nursing education program.
• Ability to follow facility policies and support quality care.
• Must meet physical job requirements, including lifting, bending, and standing.
• No history of abuse convictions.

Responsibilities

• Provide total care for residents on assigned unit during scheduled shift as an RN.
• Collaborate with allied health team members to develop and implement individualized care plans.
• Administer prescribed medications and treatments in accordance with policy and evaluate effectiveness.
• Maintain an environment that supports residents’ physical, emotional, and social well-being.
• Educate residents and families about care plans, treatments, and procedures.
• Monitor team performance and support staff development, evaluations, and corrective actions as needed.
• Assign and oversee nursing duties to ensure tasks are completed appropriately and efficiently.
• Stay current on facility procedures and participate in in-service and continuing education.
• Identify and respond to significant changes in residents’ conditions.
• Accurately document care provided, changes in condition, and other relevant information per facility guidelines.
• Act as a charge nurse and serve as a resource for staff, offering clinical support and decision-making guidance.
• Follow all facility and departmental safety protocols.
