RYZE West is seeking a dedicated LPN/RN to join our compassionate care team in our skilled nursing facility. You will provide total nursing care, supervise team members, and ensure compliance with facility standards. This role emphasizes high-quality resident care and professional development.

Responsibilities include developing care plans, administering medications, and educating residents and families. You will also act as a charge nurse, guiding staff and upholding safety policies.

Qualifications

• Graduation from an accredited nursing education program.

Duties And Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN) / Registered Nurse (RN)
• Participate in developing and implementing individualized care plans alongside the healthcare team
• Administer medications and treatments as prescribed and evaluate effectiveness regularly
• Maintain an environment that promotes residents’ physical, social, and psychological well-being
• Educate residents and families on care plans and medical procedures
• Monitor team member performance and assist with development, evaluation, and corrective action if needed
• Delegate and ensure completion of assigned nursing tasks to team members
• Stay current with facility policies, procedures, and ongoing nursing education
• Recognize and act on significant changes in resident conditions
• Document care, responses, and relevant information per facility guidelines
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
• Follow all facility safety policies and procedures

Skills And Qualifications

• Graduation from an accredited nursing education program
• Active Licensed Practical Nurse (LPN) / Registered Nurse (RN) license in the state of Illinois
