Job summary

RYZE at Homewood in Homewood, IL, is seeking an LPN/RN for PM shift to deliver comprehensive nursing care and supervise staff. The role includes administering medications, developing care plans, and ensuring compliance with clinical standards.

You will collaborate with physicians and a multidisciplinary team to provide resident-centered care and participate in shift reports and ongoing education.

Qualifications

• Graduate of an accredited nursing program.
• Current Illinois LPN or RN license in good standing.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• CPR certification preferred or required per policy.
• Strong assessment, clinical judgment, and critical thinking.
• Excellent communication, leadership, and organizational abilities.

Responsibilities

• Provide comprehensive nursing care to residents during the PM shift.

• Supervise CNAs and staff, offering guidance.
• Delegate nursing assignments and ensure quality care.
• Document assessments, treatments, and observations in EMR.
• Educate residents and family about medications and care plans.
• Serve as Charge Nurse coordinating care during the shift.
• Ensure infection control, safety, and regulatory compliance.
• Collaborate with other departments for high-quality care.
• Participate in shift reports and in-service education.
• Perform other duties as assigned.

Duties and Responsibilities

• Provide comprehensive nursing care to residents during the PM shift.
• Assess resident conditions and collaborate with the interdisciplinary team to develop, implement, and update individualized care plans.
• Administer medications, treatments, and therapies as ordered by the physician.
• Monitor residents for changes in condition and promptly communicate concerns to the appropriate healthcare provider.
• Supervise Certified Nursing Assistants (CNAs) and other assigned staff, providing guidance and support throughout the shift.
• Delegate nursing assignments appropriately and ensure quality resident care is delivered.
• Accurately document resident assessments, treatments, medication administration, and significant observations in the electronic medical record.
• Educate residents and family members regarding medications, treatments, and care plans.
• Serve as Charge Nurse by addressing clinical concerns, supporting staff, and coordinating resident care during the shift.
• Ensure compliance with infection prevention protocols, safety procedures, and all federal, state, and facility regulations.
• Collaborate with physicians, therapists, social services, dietary, and other departments to provide high-quality resident care.
• Participate in shift reports, care conferences, and required in-service education.
• Perform other duties as assigned.

Skills and Qualifications
