Job summary

Ozark Care and Rehab seeks an experienced MDS Coordinator (LPN or RN) to lead the MDS/RAI process, ensuring accurate documentation and regulatory compliance for OBRA, Medicare PPS, and Medicaid requirements.

The role involves collaboration with the Director of Nursing, training staff, and managing the MDS department while protecting resident safety and confidentiality across the facility.

Qualifications

• Knowledge of RAI, MDS, OBRA and Medicaid/State requirements.
• Experience with MDS coding and documentation for accurate patient assessments.
• Ability to train staff and collaborate with IDT and DON for care planning.

Responsibilities

• Lead the MDS/RAI process and ensure timely, accurate assessments.
• Collaborate with the Director of Nursing to ensure proper nursing documentation and risk assessments.
• Provide ongoing education to staff on RAI processes, documentation guidelines, and regulatory changes.
