Job summary

Woodland Care & Rehab in Missouri seeks a compassionate MDS Coordinator (RN or LPN) to lead RAI/MDS processes, coordinate timely nursing documentation, and collaborate with the DON to ensure accurate assessments and compliant coding.

Qualifications

• Graduate of an accredited school of nursing.
• MDS experience preferred; will train motivated candidates.
• Missouri RN or LPN license required.
• CPR certification.
• Maintains communication with employer via email/phone.

Responsibilities

• Oversee the MDS/RAI process and ensure regulatory compliance.
• Coordinate pre-admission information for Medicaid eligibility when applicable.
• Collaborate with the Director of Nursing to complete timely nursing documentation.
• Provide training and support on RAI processes to staff.
• Manage MDS assessments and ensure accurate coding and care plans.
• Attend meetings and contribute to quality and reimbursement reporting.
