Job summary

Fiesta Park Wellness and Rehabilitation is seeking a skilled MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator by ensuring documentation adheres to Federal, State, and Certification guidelines. The role coordinates the RAI process through MDS, CAAs, and the Interdisciplinary Care Plan.

The ideal candidate holds RN or LVN licensure and has experience in Skilled Nursing Facilities, with knowledge of Case Mix, OBRA, and Medicare/Medicaid reimbursement.

Qualifications

• Knowledge of Case Mix and Federal Medicare PPS processes.
• Understanding OBRA regulations and Minimum Data Set (MDS).
• Familiarity with care planning process and Medicaid reimbursement.

Responsibilities

• Assist the Director of Nursing and RN Assessment Coordinator to ensure documentation meets Federal and State guidelines.
• Coordinate the RAI process ensuring timeliness and completeness of MDS, CAAs, and Interdisciplinary Care Plan.
• Support regulatory or certification surveys and address deficiencies.
• Report trends from audits to Quality Assurance Committee.
• Monitor RAI process tracking, discharge/reentry and Medicaid tracking forms.
• Follow up with staff to ensure documentation standards are met.
• Complete patient assessments, data collection, and staff interviews as needed.
• Ensure accurate Assessment Reference Date reflecting patient care needs.
