PAM Health Specialty Hospital of Stoughton seeks an experienced Director of Case Management (RN preferred) to lead the hospital-wide case management program, oversee discharge planning, and ensure appropriate level of care across all admissions.

Relocation assistance up to $20,000 is available; the role offers competitive compensation and a comprehensive benefits package, including medical, dental/vision, EAP, 401(k), and ongoing education opportunities.

Qualifications

• RN licensure in the state where the hospital resides required.
• Social workers may be accepted in lieu of RN licensure with appropriate credentials.
• LPN, MSW, LSW, PT, OT, SLP and RT licensure may be accepted per state regulations.
• Current BLS certification required. BSN/MSN preferred. Certification in Case Management preferred.

Responsabilités

• Coordinate the hospital-wide case management program.
• Review patient records to ensure proper utilization of services.
• Coordinate timely care and discharge planning.
• Screen admissions to determine level of care.
• Supervise case managers and daily operations.
• Support fiscal planning and regulatory compliance.
• Collaborate with interdisciplinary team to improve quality of care.
