Job summary

Polaris Rehabilitation & Care Center in Cheyenne, Wyoming, seeks a dedicated Registered Nurse (RN) to lead direct resident care and act as a clinical resource during shifts. You will oversee assessments, medications, and coordination of care with physicians and the interdisciplinary team.

The role emphasizes hands-on resident care with leadership responsibilities, mentoring nursing staff, and ensuring safe, individualized care plans. Sign-on bonus available for eligible full-time RNs.

Qualifications

• Graduate of an accredited nursing program.
• Current, active RN license in Wyoming in good standing.
• Current CPR or BLS certification, as required.
• Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric nursing preferred.
• Strong clinical assessment and nursing judgment skills.

Responsibilities

• Provide and oversee nursing care according to care plans and clinical standards.
• Conduct nursing assessments and monitor residents for changes.
• Safely administer medications and treatments within RN scope.
• Supervise and coach CNAs and nursing staff during shifts.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Maintain accurate EMR documentation.
• Report urgent changes and ensure safe resident transitions.
