Polaris Rehabilitation & Care Center in Cheyenne, Wyoming is seeking compassionate CNAs to join our nursing team. CNAs provide hands-on care, assist with ADLs, mobility, and personal care while collaborating with the nursing staff to deliver safe, respectful, resident-centered care.

Qualifications

• Must hold current Wyoming CNA certification in good standing.
• Experience in skilled nursing, rehabilitation, long-term care, or assisted living preferred but not required.
• Strong communication and interpersonal skills are essential.

Responsibilities

• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, feeding and personal hygiene.
• Obtain and record vital signs (temperature, pulse, respirations, blood pressure) accurately.
• Observe residents for changes in condition and report concerns promptly to the supervising nurse.
• Assist with transfers, ambulation, repositioning and mobility equipment per care plans and safety procedures.
• Respond promptly to call lights and resident requests.
• Assist with meal service and provide feeding assistance when needed.
• Provide routine skin care, incontinence care, oral care, and other CNA duties.
• Document care in facility procedures accurately.
• Follow resident care plans and communicate any concerns to licensed staff.
• Maintain clean, safe, and organized rooms and care areas.
• Follow infection control, fall prevention, and safety procedures.
• Respect resident privacy, dignity, and rights.
• Communicate respectfully with residents, families, visitors, nurses, and team members.
