Job summary

Sanford Health in Fargo, ND seeks an RN Transplant Coordinator to coordinate care between the transplant center and major patient referral sources for renal transplantation. You will manage education, pre/post-transplant workups, and activation planning for transplantation.

The role includes a designated caseload, coordinating referrals, addressing roadblocks, and delivering patient, family, and staff education while ensuring compliance with governing agencies.

Qualifications

• Bachelor's degree in nursing required.
• Graduate from an accredited nursing program (AACN, ACEN, NLN CNEA).
• Bachelor's Degree in nursing, or in process with expected completion within five years.
• Currently holds an unencumbered RN license where practice occurs; multistate licensure if in an NLC state.
• Minnesota RN license required within six months of date into position.
• Maintains required department competencies and certifications.

Responsibilities

• Coordinate transplant center and referral sources for renal transplantation.
• Provide education and coordination of care pre/post transplant.
• Manage a designated patient caseload and referrals; address roadblocks in the workup process.
• Deliver patient, family, and staff education; ensure regulatory and program compliance.
• May provide outreach to regional dialysis and nephrology programs.
