Job summary

Hackensack Meridian Health is seeking an experienced RN to support the transfer center, coordinating inter-facility transfers and ensuring accurate documentation in the electronic health record. You will collaborate with capacity management, nursing, and providers to assure appropriate care and bed assignments.

The role requires at least 2 years in acute care, NJ RN licensure, and strong communication. BSN preferred; hospital setting experience is highly valued.

Qualifications

• RN graduate of an accredited school of nursing.
• Minimum of 2 years clinical experience in acute care setting.
• BSN preferred.
• Transfer center experience preferred.
• Critical care and emergency medical services experience preferred.
• Computer literacy and excellent communication skills.

Responsibilities

• Answers telephones in accordance with established policies and procedures.
• Coordinates communication between sending and receiving facilities, and within the receiving facility.
• Accurately records details of all communication related to each transfer request in the electronic health record.
• Works in a collaborative manner with other members of the health care team.
• Collaborates with capacity management, nursing, physician/provider representatives, and other departments to assure proper level of care and room assignment.
• Maintains awareness of bed availability and initiates registrations and handoffs as directed.
