Qualifications

• Current and valid New Jersey Registered Nurse license.
• RN graduate of an accredited school of nursing.
• BLS/CPR certification by American Heart Association.
• At least 2 years of clinical experience in an acute care setting.
• BSN preferred.

Responsibilities

• Answers telephones in accordance with established policies and procedures.
• Coordinates communication between sending and receiving facilities, and within the receiving facility.
• Accurately records details of all communication related to transfer requests in the electronic health record.
• Collaborates with capacity management, nursing, physician/provider representatives, and other departments to assure proper level of care and room assignment.
• Maintains awareness of bed availability across units involved in transfers and direct admissions.
• Mentors team members and participates in onboarding of new staff.
• Follows patient identification policies and collaborates with patient access services as needed.
