Job summary

Hackensack Meridian Health is seeking a Clinical Coordinator in New Jersey to facilitate inter-facility transfers, coordinate communications, and document encounters in the electronic health record. You will work with EMS, nursing, physicians, and care teams to ensure appropriate level of care and bed assignment across units.

The role requires an active New Jersey RN license, 2+ years acute care experience, and BLS/CPR certification. Transfer center experience is preferred; BSN is a plus.

Qualifications

• Current and valid New Jersey Registered Nurse license is required.
• RN graduate of an accredited school of nursing.
• BLS/CPR certification by American Heart Association is required
• At least 2 years of clinical experience in an acute care setting required
• Critical care and/or emergency nursing experience preferred
• Transfer center experience preferred
• BSN preferred

Responsibilities

• Answers telephones in accordance with established policies and procedures.
• Coordinates communication between sending and receiving facilities, and within the receiving facility.
• Accurately records details of all communication related to each transfer request in the electronic health record.
• Maintains awareness of bed availability on all patient care units and outpatient departments.
• Escalates barriers and concerns using applicable policies and procedures as well as chain of command.
• Prepares handoff and other reports as directed.
