Qualifications

• RN graduate from an accredited school of nursing.
• Minimum of 2 years clinical experience in an acute care setting.
• NJ State Professional Registered Nurse License.
• BLS/ACLS or AHA Basic Life Support for Healthcare Providers.

Responsibilities

• Facilitate communication between sending and receiving facilities.
• Document a record of each encounter.
• Answer telephones in accordance with established policies.
• Accurately record details of all communication in the EHR.
• Collaborate with capacity management, nursing, and physician/provider representatives.
• Maintain awareness of bed availability across patient care units.
• Diffuse challenging situations and respond appropriately.
• Mentor team members and participate in onboarding new staff.

About This Role

The Clinical Coordinator for the Transfer Center is responsible for facilitating communication between sending and receiving facilities, utilizing clinical expertise and critical thinking to ensure proper patient care and documentation. This role is ideal for experienced nurses who thrive in a collaborative environment and can handle high-pressure situations.

What You'll Do

• Facilitate communication between sending and receiving facilities.
• Document a record of each encounter.
• Answer telephones in accordance with established policies.
• Accurately record details of all communication in the electronic health record.
• Collaborate with capacity management, nursing, and physician/provider representatives.
• Maintain awareness of bed availability across patient care units.
• Handle high-pressure situations and establish priorities for effective work completion.
• Diffuse challenging situations and respond appropriately.
• Collaborate with Emergency Medical Services (EMS) for timely transportation.
• Mentor team members and participate in onboarding new staff.

Requirements

• Education: RN graduate of an accredited school of nursing
• Experience: Minimum of 2 years clinical experience in acute care setting
• Licenses & Certifications: NJ State Professional Registered Nurse License
• Licenses & Certifications: BLS AHA Basic Health Care Life Support HCP Certification
