ChenMed seeks an Intensive Community Care Manager (ICCM) RN to lead high‑risk patient care within our Complex Care Team in Philadelphia. This on-site role involves in-home visits, coordination with primary care physicians, and clinical supervision to keep patients safely at home and engaged in care.

• Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

Responsabilidades

• Provides in-house/at-facility and home visits to high-risk patients to prevent hospital admissions.
• Coordinates and conducts case management and care planning with PCPs and care team members.
• Provides supervision and guidance to LPNs and other team members as needed.
• Performs health screenings, medication reviews, and education in outpatient/home settings.
• Coordinates and documents multidisciplinary team meetings and ensures smooth handoffs.
• Supports discharge planning and patient safety initiatives in the Complex Care program.
