Job summary

ChenMed LLC is seeking a nurse leader to serve as Intensive Community Care Manager (ICCM). This on-site role requires daily home visits to our most complex patients, coordinating with PCPs and the care team to maintain patients at home and reduce hospitalizations.

The ICCM will mentor team members, develop comprehensive care plans, and lead clinical activities. Strong communication, clinical judgment, and travel within the service area are essential.

Qualifications

• Active RN license in state of employment.
• Compact license for MD or nearby states preferred.
• Minimum 2 years of clinical work experience.
• Case management experience is highly desired.

Responsibilities

• Provides in-home, facility, and telephonic visits to high-risk patients to prevent hospitalizations.
• Develops and oversees care plans with the care team and PCPs.
• Supervises LPNs and ensures proper discharge from case management.
• Performs SdoH assessments, medication monitoring, and health education.
• Coordinates multidisciplinary team meetings and patient navigation of health care services.
