Presbyterian Espanola Hospital, part of Presbyterian Healthcare Services, is seeking an RN Case Manager to provide clinically based case management and care coordination across the continuum of care in Espanola, NM. You will partner with physicians, patients and payers to optimize length of stay, discharge planning, and transitions.

Qualifications

• Associates Degree in nursing required.
• BSN within 5 years of hire date.
• Current State of New Mexico RN licensure required.
• 2 years clinical nursing experience with utilization review or case management desirable.
• National Case Management certification preferred.

Responsibilities

• Manage a designated caseload across the continuum of care.
• Coordinate utilization management and status determination.
• Lead discharge planning and care transitions.
• Collaborate with physicians, nursing, social work and post-acute providers.
• Maintain accurate, timely documentation and support quality improvement.
