Job summary

Presbyterian Espanola Hospital in Espanola, NM is hiring a RN Case Manager to provide clinically-based case management, utilization management, care coordination, and discharge planning.

You will manage a designated caseload, collaborate with an interdisciplinary team, and work with physicians, patients, families, payers, and post-acute providers to improve outcomes and control costs.

Qualifications

• RN IMM Case Management I: Associates degree in nursing required; BSN within 5 years; NM RN license.
• RN IMM Case Management II: BSN or ADN + 5 years utilization review/case management; BSN within 5 years; NM RN license; 2 years adult bedside experience; CCM preferred
• RN IMM Case Management III: BSN + 5 years or MSN; NM RN license; 2 years bedside; CCM preferred
• RN IMM Case Management IV: MSN or BSN +10 years; NM RN license; 2 years bedside; CCM preferred

Responsibilities

• Manage a designated caseload across the continuum of care.
• Coordinate utilization management and status determination.
• Lead discharge planning and care transitions; arrange post-acute services.
• Collaborate with physicians, nursing, social work, and ancillary services.
• Maintain accurate documentation and support performance improvement.
