Job summary

Presbyterian Espanola Hospital is hiring an RN Case Manager to provide clinically-based case management supporting effective patient care. You will coordinate discharge planning, utilization management, and transitions across the continuum, partnering with physicians, patients, families, payers, and post-acute providers.

You will manage a designated caseload focusing on reducing LOS, improving outcomes, and ensuring timely information for payers and care teams.

Qualifications

• AA in nursing required; BSN preferred within 5 years.
• Current NM RN licensure required.
• Experience in utilization review or case management desirable.
• National Case Management certification preferred.

Responsibilities

• Manage a designated caseload and optimize length of stay.
• Coordinate discharge planning and safe transitions of care.
• Collaborate with physicians, payers, families, and post-acute providers.
• Assist with utilization management and documentation to support care quality.
• Engage in care conferences and rounds with the interdisciplinary team.
