Qualifications

• 12 months nursing experience required; critical care experience preferred.
• BLS within 90 days of hire.
• Associates of nursing required; BSN is preferred.
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
• 1-year acute care case management experience preferred.
• Experience with community resources preferred.

Responsibilities

• Provide and facilitate comprehensive care management across the healthcare continuum.
• Focus on patient transitions to appropriate levels of care and clinical appropriateness.
• Develop and implement comprehensive care plans addressing physical, mental, and psychosocial needs.
• Collaborate with the healthcare team and patient/caregiver to manage care and transitions.
