Sentara Healthcare Inc. is seeking an Inpatient Case Management Supervisor, RN to lead our case management teams across Sentara CarePlex Hospital in Hampton and Sentara Louise Obici Hospital in Suffolk.

This full-time, day-shift role emphasizes patient advocacy, staff development, and operational excellence in a high-acuity setting. You will oversee discharge planning, care coordination, and length-of-stay initiatives while coaching staff and ensuring compliance with policies.

Qualifications

• BSN degree required.
• Current RN licensure valid in Virginia or Compact/Multi-State.
• BLS certification required within 90 days of hire.
• Specialty certification (ACM/ CCM/ CCCTM/ CMAC/ CMGT-BC) within one year.
• Minimum 3 years of inpatient case management experience.
• Previous supervisory experience preferred.

Responsibilities

• Supervise day-to-day case management across two Sentara facilities (CarePlex and Obici).
• Ensure effective discharge planning and patient flow.
• Support length-of-stay management and performance improvement initiatives.
• Provide coaching and mentorship to staff.
• Collaborate with leadership and HR on employee relations.
• Ensure compliance with department standards and hospital policies.
