Job summary

Sentara Northern Virginia Medical Center in Woodbridge, VA, seeks an Inpatient Case Manager – RN for a full-time day shift. The role focuses on coordinating care transitions, ensuring clinical appropriateness, and optimizing length of stay. Weekday schedule with some weekends/holidays rotation. 40 hours weekly.

Responsibilities include developing care plans, coordinating services, and supporting patients and families. Requires CCM/CMAC/ACM-SW within one year, MSW, and BLS within 90 days.

Qualifications

• Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility.
• Minimum of 1 year of acute care and/or healthcare experience; internship may be considered.
• BLS certification required within 90 days of hire.
• MSW (Master’s Level Degree in Social Work) required.

Responsibilities

• Develop and implement comprehensive care plans based on assessment and evaluation for inpatients and caregivers.
• Coordinate and monitor delivery of health services to meet clinical goals.
• Facilitate transitions to appropriate levels of care with timely discharge planning.
• Advise and support care teams, patients, and families regarding plans and resources.
