Job summary

Sentara Norfolk General Hospital in Norfolk, VA is seeking an Inpatient Case Manager RN to oversee comprehensive care management for inpatients, with a focus on safe and timely transitions across the care continuum.

The role requires an RN license in VA/NC or multi-state, a Bachelor’s degree in Nursing (preferred), and CM certification within one year. Work hours are 8:00 AM–4:30 PM with rotating weekends; onsite at 600 Gresham Drive.

Qualifications

• Bachelor’s Degree in Nursing preferred.
• Registered Nurse (RN) license in VA or NC or Compact/Multi-state required.
• BLS certification within 90 days of hire.
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification within one year of eligibility.

Responsibilities

• Coordinate comprehensive care management for inpatients across the healthcare continuum.
• Support patient transitions to appropriate levels of care and ensure clinical appropriateness and LOS.
• Develop and implement a care plan addressing physical, mental and psychosocial needs of patients and caregivers.
