Qualifications

• 1 year of leadership experience in a healthcare setting.
• 3 years of Case Management experience.
• Virginia or Compact Multi-State License in good standing; BLS within 90 days of hire.
• Specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CMGT-BC).

Responsibilities

• Lead inpatient case management operations for the medical center.
• Collaborate with RNs, MSWs, and support staff to optimize patient care and discharge planning.
• Promote Magnet-recognized hospital standards and shared governance practices.
• Drive quality outcomes and ensure compliance with regulatory and organizational policies.
