Job summary

Wake Forest Baptist Medical Center is seeking a full-time nurse in Care Coordination to conduct comprehensive assessments, plan care, and initiate timely interventions during the hospital stay.

You will collaborate with patients, families, social work and medical staff to coordinate discharge planning, utilization management and referrals, ensuring regulatory compliance and smooth transitions. The role requires RN licensure and prefers BSN, with a 40-hour week, Monday–Friday 8am–5pm.

Qualifications

• Graduate of a Board of Nursing approved nursing education program (RN).
• BSN preferred.

Responsibilities

• Conducts complete assessments, establishes plans, and initiates interventions within desired timeframes.
• Provides discharge planning, care coordination, and utilization management activities with regulatory compliance.
• Collaborates with Social Work, physicians, and care teams to ensure timely progression of care and transitions.
• Documents discharge planning interventions and maintains timely communication with payers and external review organizations.
