Job summary

Mercy Medical Center in Baltimore is seeking an RN Case Manager to join our team. This role will assess, plan, implement, and coordinate care, working with physicians and social workers to promote quality, cost-efficient outcomes.

Responsibilities include managing inpatient LOS, denied days, and discharge planning, with daily utilization reviews. Candidates must have a nursing degree and an active RN license, with 3 years’ clinical experience preferred.

Qualifications

• Must possess a nursing degree.
• Active RN license required.
• 3 years of clinical experience preferred in a medical/surgical setting.
• Managed care experience preferred.
• Case management experience preferred.
• Utilization review skills using MCG preferred.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate patient care to meet health needs.
• Oversee inpatient length of stay, denied days, resources utilization and discharge planning.
• Coordinate with physicians, social workers and staff; advocate for patient rights and resources.
• Provide daily utilization reviews and crisis intervention when necessary.
