Job summary

Molina Healthcare is seeking a Texas licensed RN to join our care management team serving Medicare members in southern Dallas County. Care managers meet members in their homes, complete assessments, and determine services. Mileage is reimbursed as part of our benefits package, with hours Monday–Friday, 8 AM–5 PM CST.

The role emphasizes collaboration with multidisciplinary teams, care plans, and coordination of LTSS to deliver quality and cost-effective member care.

Qualifications

• Active RN license in state of practice.
• 2+ years health care experience incl. care management or managed care.
• Experience with LTSS, disabilities, chronic conditions, or related fields is preferred.
• Valid driver's license and reliable transportation for travel to member homes.

Responsibilities

• Complete comprehensive member assessments, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document interventions.
• Coordinate services across behavioral health, LTSS, and community resources.
• Identify barriers to care and address psycho/social, financial, and medical needs.
