Job summary

Molina Healthcare seeks a licensed LVN/LBSW/LMSW to join our LTSS Team as a Case Manager serving Medicaid members in the southern Dallas County area. You will conduct in-home assessments, coordinate services, and travel 25–40% as required.

Work with a multidisciplinary team to develop and monitor care plans, ensure regulatory compliance, and promote quality, cost-effective member care. Strong MS Office skills and communication are essential.

Qualifications

• 2+ years of health care experience, including LTSS and care management or equivalent combination of education/experience.
• License must be active and unrestricted in state of practice if required by contract or regulation.
• Valid and unrestricted driver’s license, reliable transportation, and auto insurance for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision and manage multiple priorities.

Responsibilities

• Complete comprehensive member assessments, including in-home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers and clinicians.
• Monitor care plan effectiveness and document interventions and outcomes.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize appropriate waiver services.
• Collaborate with the interdisciplinary care team for service approvals.
• Utilize motivational interviewing and Molina guidelines to support change.
• Identify barriers to care and coordinate solutions for psycho-social and financial obstacles.
• Coordinate travel and meet regulatory timelines for member care.
