Job summary

Molina Healthcare is seeking a Texas-licensed RN to join our care management team serving Medicare members in southern Dallas County. The role focuses on home visits, waiver coordination, and care plan development with a multidisciplinary team.

Flexibility for travel and adherence to timelines is essential. Ideal candidates will have prior Medicaid/Medicare experience within an MCO, strong Microsoft Office skills, and the ability to navigate multiple programs for documentation and forms.

Qualifications

• At least 2 years of experience in health care, including 1 year in care management or managed care.
• RN license active and unrestricted in practicing state.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and staff.
• Monitor care plan effectiveness and document outcomes.
• Promote integration of services across LTSS and behavioral health.
• Identify barriers to care and coordinate solutions.
• Collaborate with ICT meetings for service approvals/denials.
• Potentially provide medication reconciliation.
• Travel 25–40% as required.
