Job summary

Molina Healthcare, Inc. is hiring a Texas-licensed RN as a Field Care Manager to support Medicaid members in Arlington, TX. You will assess needs, coordinate long-term services, and work with a multidisciplinary team to improve outcomes while managing costs. Travel within 30–45 minutes of the coverage area is required.

Hours are Monday–Friday, 8 AM–5 PM CST. Proficiency with Microsoft Office is essential, and prior Medicaid/Managed Care exposure is preferred.

Qualifications

• RN with active license in state of practice.
• 2+ years health care experience including care management.
• Valid driver's license and reliable transportation.
• Proficient with Microsoft Office.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits as required.
• Facilitates enrollment and disenrollment processes for waivers.
• Develops and implements care plans with members and providers.
• Monitors care plans and documents interventions and outcomes.
• Promotes integration of services across LTSS and behavioral health.
• Assesses medical necessity and authorizes waiver services.
• Coordinates ICT meetings for service approvals.
• Uses motivational interviewing to educate and motivate members.
