Molina Healthcare, Inc. in Texas is seeking an experienced Field Care Manager (RN) to support Medicaid members in the Dallas area, including Lancaster, DeSoto, Duncanville, and Red Oak. The role involves home visits, member assessments, care planning, and coordination with a multidisciplinary team.

Travel up to 25-40% may be required. Candidates with Medicaid/MCO experience are preferred. Proficiency in Microsoft Office is essential, with strong communication and organizational skills.

Competenze

• RN license active and unrestricted in state of practice.
• At least 2 years of health care experience, including 1 year in care management/managed care and 1 year with LTSS/populations.
• Valid and unrestricted driver’s license and adequate auto insurance for job-related travel.
• Proficiency with Microsoft Office suite (Outlook, Excel, OneNote, Teams).

Mansioni

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and health professionals.
• Monitor care plan effectiveness and document interventions and outcomes.
• Promote integration of LTSS with behavioral health and home/community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings for service approvals.
