Job summary

Molina Healthcare is seeking a Texas-licensed RN to join as a Field Care Manager supporting Medicaid members in the Mission service area. This role emphasizes comprehensive assessments, waiver enrollment, and coordinated LTSS care planning with a multidisciplinary team.

Ideal candidates have 2+ years in health care, including care management, and a valid driver's license for local travel. Hourly pay range and mileage reimbursement are part of the benefits package.

Qualifications

• Minimum 2 years experience in health care, with at least 1 year in care management or managed care.
• RN license active and unrestricted in state of practice.
• Bachelor's degree may be required in some states; dependent on state/contractual requirements.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Develop and implement care plans including waiver service plans with members and care partners.
• Monitor care plan effectiveness and adjust as needed to meet outcomes.
• Coordinate with interdisciplinary teams to ensure integrated member care across LTSS and behavioral health services.
• Travel locally 25-40% as required by state/contractual obligations.
