Qualifications

• Active RN license in good standing in the state of practice.
• Experience in skilled nursing or long-term care is preferred but not required.

Responsibilities

• Supervise and support CNAs under the Director of Nursing.
• Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate care plans.
• Ensure initial baseline and periodic assessments are completed on time.
• Prepare for physician rounds and document findings for review.
• Initiate referrals based on resident needs.
• Perform medical documentation, evaluations, and reports.
• Deliver nursing care within scope of practice and facility policy.
• Monitor residents for changes and notify providers as needed.
• Perform other duties as assigned by the Director of Nursing.
