Responsibilities

• Acts as the primary clinical coordinator for the Ambulatory Hypertension Management Program, partnering with primary care providers, specialists, pharmacists and population health teams.
• Proactively manages patients with uncontrolled hypertension through education, monitoring, outreach, and care coordination.
• Documents nursing history and assessment, initiates referrals based on patient needs.
• Performs nursing interventions and supports patient safety and comfort during ambulatory visits.
• Collaborates with healthcare providers to develop individualized care plans and patient care pathways.
• Recruits patients for studies and assists with quality assurance initiatives.
