Job summary

Qualifications

• Associate's degree in nursing required; Ohio RN licensure in good standing.
• Bachelor's degree in nursing preferred.
• Current Ohio RN license; BLS certification preferred.
• Minimum of 2 years in ambulatory or outpatient nursing.

Responsibilities

• Provide direct patient care and care coordination in ambulatory clinics.
• Develop and execute individualized care plans with patients.
• Educate patients and families on health conditions and self-care.
• Collaborate with interdisciplinary care teams to optimize outcomes.
• Conduct nursing assessments and document findings.
• Assist with transitions of care and referrals.
• Maintain timely, accurate nursing documentation.
• Participate in quality improvement and safety initiatives.
