Responsibilities

• Develop and execute individualized care plans with the care team.
• Document nursing assessments and coordinate referrals and follow-ups.
• Facilitate transitions of care and ensure timely access to services.
• Collaborate with physicians, social workers, dietitians and other healthcare professionals.
