Qualifications

• Nurse must have an active Ohio RN license.
• Minimum 2 years of nursing experience in ambulatory or outpatient settings.
• BLS certification preferred.

Responsibilities

• Coordinate clinical care for ambulatory hypertension program.
• Educate patients on blood pressure management and adherence.
• Collaborate with primary care providers, specialists, pharmacists, and population health teams.
• Perform and document nursing assessments and care plans.
• Identify and respond to patient safety concerns and changes in conditions.
• Assist with referrals, scheduling, and transitions of care within ambulatory settings.
