Job summary

The Knollwood Nursing Center within The Briarwood Community in Worcester, MA seeks a part-time Hospital Liaison/Screener. You will evaluate prospective patients at area hospitals for admission to Knollwood’s sub-acute unit and assess clinical needs, resources, and reimbursement considerations to support appropriate placement and high-quality outcomes.

Qualifications

• Current RN, LPN, LICSW, or LCSW credentials.
• At least one year of experience in long-term care and hospital/geriatric services.
• Experience with patient interviewing, assessment, and evaluation.
• Strong interpersonal and communication skills with residents, families, and hospital staff.
• Proficiency with computers, including MS Word.
• Ability to travel between Knollwood and area hospitals.

Responsibilities

• Evaluate patients at area hospitals for admission to the sub-acute unit.
• Assess patient needs and care requirements for appropriate placement.
• Review patients relative to HMO contracts and reimbursement.
• Consult with nursing and therapy leadership to determine placement.
• Complete required clinical and administrative documentation.
• Coordinate and communicate with hospital staff and families throughout the referral process.
• Represent Knollwood at hospital and HMO meetings.
• Build and maintain relationships with area hospitals and centers.
• Conduct marketing and outreach activities with area hospitals and rehabilitation centers to promote Knollwood’s sub-acute services.
• Communicate evaluation findings and recommendations clearly and effectively, both verbally and in writing.
