Intermountain Health seeks an RN Hospital Care Manager I to deliver comprehensive care management for designated patients. The role uses clinical expertise for screenings, assessments, and evaluations, developing patient-centered care plans with shared goals across inpatient and outpatient settings.

Formación

• Current Registered Nurse (RN) license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years if not already.
• Demonstrated inpatient clinical nursing experience.
• Understanding of disease management and barriers to delivery of care.
• Preferred: Case management experience, utilization review, and discharge planning.

Responsabilidades

• Assessment: Identifies patients using screening criteria and conducts comprehensive evaluations.
• Care Planning: Develops, adjusts, and coordinates care plans addressing medical and social needs.
• Patient Support: Guides self-management using motivational interviewing and coaching.
• Education & Advocacy: Educates care teams and patients, advocates for patient rights.
• Collaboration: Works with multidisciplinary teams for timely, high-quality care.
• Process Improvement: Leads initiatives to enhance care transitions and policy adherence.
• Advanced Care Planning: Provides Goals of Care education for informed decisions.
• Data Analysis & Compliance: Tracks metrics, analyzes trends, ensures regulatory compliance.
