Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse (RN) to join our Admissions Team serving Broward County. This on-site role supports patients and families through one of life's significant transitions, with a schedule of 12pm–10:30pm and every other weekend.

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support certification.
• Experience in public health, home health, oncology, medical/surgical, geriatric or Hospice nursing preferred.
• Proficiency with EMR software is a plus.

Responsibilities

• Evaluate patients referred for hospice services.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Admissions Team serving Broward County. This is a meaningful opportunity for an RN who is passionate about guiding patients and families through one of life's most significant transitions with empathy, clarity, and dignity.

You will work the following schedule: 12pm - 10:30pm and every other weekend.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What You’ll Do

• Serve as the primary nurse responsible for evaluating patients referred for hospice services
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice-appropriate plans of care
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long-term care facilities, and other healthcare professionals
• Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community
• Your work ensures patients receive timely, compassionate care while helping families feel informed, supported, and confident in their hospice journey.

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Possess the ability to assess physical and psychosocial needs of patient and family.
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
