Job summary

Providence Hospice, a division of Providence Life Services, seeks an RN Hospice Case Manager in St. John, IN. You will provide skilled nursing and case management to patients and families at end of life, traveling to patients' homes or facilities while partnering with an experienced hospice team.

The role requires an Indiana RN license, at least 1 year of hospice or home health experience, and a valid driver’s license. A comprehensive benefits package and retention bonus are offered.

Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County. We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. $5,000 retention bonus now being offered. Retention Bonus is paid out in increments when milestone hours are met.

Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly.
