Job summary

Providence Hospice, a division of Providence Life Services, seeks an RN Hospice Case Manager for a full-time role in St. John, IN, and throughout Lake County.

You will provide compassionate skilled nursing and case management, with on-call night coverage and travel to patient locations. The position requires an active Indiana RN license, 1+ year hospice or home health experience, and reliable transportation.

Qualifications

• Active RN license in Indiana.
• At least 1 year of RN experience, preferably in Hospice or Home Health.
• Driver's license in good standing.
• Reliable transportation to travel to patients' homes and facilities.

Responsibilities

• Provide skilled nursing care.
• Provide case management services.
• Conduct initial assessments of new patients.
• Travel to patients wherever they reside: home, nursing facility, etc.
