Job summary

Caretenders Hospice, part of the Optum family, is hiring an RN Case Manager to oversee Plan of Care and coordinate hospice services. You will work with the patient, family, and IDG to ensure compassionate, comprehensive care across physical, psychosocial, emotional, and spiritual needs.

Responsibilities include initial evaluations within 48 hours, developing and coordinating the plan of care, and maintaining accurate clinical notes while supporting families during difficult times.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Preferred: Current CPR Certification or ability to complete within 90 days of hire.
• Demonstrated ability to work independently.
• Proven good communication, writing, and organizational skills.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members on daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
