Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice RN Case Manager to join our north east Broward County team. Mon-Fri 8:30am–5:00pm with weekend coverage, coordinating care under the IDG and primary physician to support patients and families through end-of-life journeys.

You will monitor conditions, provide interventions, and educate to enhance comfort and dignity. Strong advocacy, empathy, and communication are essential in this mission-driven role.

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• BLS/CPR certification is assumed where required.

Responsibilities

• Coordinate care under the primary physician and IDG.
• Develop and implement individualized care plans.
• Monitor patient conditions and provide interventions.
• Educate patients and families to improve comfort and quality of life.
• Advocate for patient and family needs and address concerns promptly.
