As a key member of our team, you will complete evaluations, conduct home visits, and develop care plans that address patient needs. Ideal candidates will have an RN license, effective communication skills, and a commitment to community health.

We offer a competitive salary range of $88,000.00 - $94,000.00 per year, along with comprehensive benefits including health insurance, generous PTO, and tuition reimbursement opportunities.

Qualifications

• Must hold a current, valid RN license through the State Board of Nursing.
• Community health/hospice experience is preferred but training is available.
• Must be a licensed driver with an insured automobile.
• Maintains current CPR certification.

Responsibilities

• Complete hospice evaluations as requested.
• Conduct in-home visits; assess patient conditions.
• Develop care plans based on diagnosis and needs.
• Administer medications as prescribed by physician.
• Communicate patient needs with the team.
