CompassionCare Hospice in Las Vegas is seeking a dedicated Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities, explain services, assess eligibility, and coordinate start of services, while collaborating with an interdisciplinary team.

We support 1:1 patient care, flexible scheduling, and a stable, growth-oriented environment with excellent benefits, including health plans, retirement, PTO, and mileage reimbursement.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year hospice nursing experience required; 3+ years preferred or similar settings.
• Possess and maintain CPR certification.
• Must have reliable automobile and valid driver's license.

Responsabilités

• Visit patients in homes, hospitals, or facilities referred for hospice care.
• Explain hospice services, discuss goals with patients and families, obtain consents and documents.
• Obtain medical history, current medications and symptoms.
• Perform clinical assessments to determine hospice eligibility per Medicare/insurance guidelines.
• Coordinate start of services and collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for h ospice care
• Explain hospice services, discuss goals and care with the patient and families , answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required , 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
