Job summary

COMPASSION CARE HOSPICE in Las Vegas, NV is seeking a passionate Hospice RN Case Manager to provide 1:1 patient care and develop individualized care plans for patients at a crucial time in their lives.

You will deliver expert, patient-centered hospice care, work within a supportive team, and enjoy a stable environment with opportunities for advancement. This role requires a valid RN license and CPR certification, with a focus on compassionate care in a home setting.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
