Job summary

Suncoast Hospice, a member of Empath Health, seeks an RN Case Manager to support North Pinellas County in private homes. Monday–Friday daytime hours, direct patient care, and care coordination with the Interdisciplinary Team.

You will assess patients, develop plans of care, supervise LPNs/HHA, and ensure compassionate, Full Life Care. Florida RN license and driving are required; bilingual Spanish/English preferred.

Qualifications

• Must have RN licensure in Florida and valid driver's license.
• Minimum of one year related experience in hospice or acute/post-acute care preferred.
• Ability to obtain patient history and perform comprehensive assessments.
• Provide patient and family education and coordinate with the IDT.

Responsibilities

• Obtain patient history, perform age-specific assessments, and initiate referrals.
• Develop and update Plan of Care with patient, family and IDT.
• Coordinate care with physicians, facilities and home health staff.
• Supervise LPNs and Home Health Aides; monitor medication administration.

Suncoast Hospice, a member of Empath Health, is seeking an RN Case Manager to join the Empath team. Monday - Friday, daytime hours. Coverage area includes North Pinellas County, providing direct patient care in private homes. As an RN Case Manager, you'll build meaningful relationships with patients and families, guiding them through their hospice journey with compassion, expertise, and dignity.Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.What you'll DoDemonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.Following a patient’s death, assists with closure contact and support in collaboration with IDT.Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician.
