Job summary

Suncoast Hospice, a member of Empath Health, is seeking a PRN Hospice RN Case Manager to join the Empath team. Providing direct patient care in homes and facility settings, you will build meaningful relationships with patients and their families, offering compassionate guidance, clinical expertise, and support throughout the hospice journey.

Requires an active Florida RN license and Associate's degree or equivalent, plus a valid Florida driver's license.

Qualifications

• Ability to obtain patient history, perform age-specific assessments, and initiate referrals with IDT collaboration.
• Develop a Plan of Care with patient/family/IDT/physician and evaluate progress.
• Assist with closure contact after a patient’s death in coordination with IDT.
• Direct and supervise LPN and Home Health Aide per agency policy; biweekly supervision visits.
• Administer and document medications as prescribed; provide patient/family teaching.
• Field-based travel requires reliable transportation and valid auto insurance.

Responsibilities

• Provide direct patient care in homes and facility settings as part of hospice services.
• Coordinate care with the IDT and attending physician; update the Plan of Care as needed.
• Monitor and evaluate progress toward patient/family goals; adjust plan accordingly.
• Support families throughout the hospice journey with guidance and care.
• Supervise LPN and Home Health Aide; ensure quality of services provided.
